PRIVACY POLICY
Effective August 16, 2016

Community Health Network, Inc. (“CHNw”) is sensitive to the concerns that you have about the use of the information you disclose to CHNw via its careers website (the “Site”). Accordingly, this Privacy Policy (the “Policy”) sets forth CHNw’s privacy practices and the possible uses of the information CHNw gathers through the Site.

Collection of Information

In connection with your use of the Site, CHNw may request and/or collect certain personal information from you, which may include, without limitation, your name, email address, home and business address, phone number and other information (“Your Information”). By entering Your Information via the Site or otherwise providing Your Information to CHNw, you consent to CHNw’s use of Your Information for purposes discussed herein.

CHNw will not sell, share, or rent Your Information to others in ways different from those disclosed in this Policy. CHNw does not intend to collect information from children under 13 years of age. If CHNw discovers that it has inadvertently collected information from a child under 13 years of age, CHNw will promptly take all reasonable measures to delete that data from its systems.

Use of Information

CHNw will generally use Your Information in connection with evaluating you as a potential employee or contractor of CHNw. CHNw will also use Your Information to communicate with you about employment opportunities and provide other information that CHNw believes might be of interest to you.
Information Sharing

CHNw will share Your Information with agents, contractors or partners in connection with services that these individuals or entities perform for, or with, CHNw. For example, CHNw may provide Your Information to agents, contractors or partners for evaluating potential employment candidates, for hosting CHNw’s databases, for data processing services, or for sending materials to individuals who contact CHNw via the Site. These agents, contractors or partners are restricted from using Your Information in any way other than to provide services for CHNw. CHNw may also disclose Your Information to its affiliated companies. These affiliated companies use of the Your Information will be consistent with this Policy.

CHNw reserves the right to disclose Your Information in the following situations: (1) in response to duly authorized information requests of governmental authorities; (2) where required by law; (3) if necessary for fraud protection and credit risk reduction purposes; (4) in the good-faith belief that such action is necessary to protect and defend the rights or property of CHNw or the users of the Site; (5) to act under urgent circumstances to protect the safety of CHNw or its employees or a member of the public; and (6) to a third party in connection with the sale, assignment, or other transfer of the business of CHNw to which the information relates, in which case CHNw will require any such buyer to agree to treat Your Information in accordance with this Policy.

Effective Date/Notification of Changes

Please note that CHNw may update this Policy from time to time, and you should check this page periodically for changes. If changes are made, CHNw will notify users by stating the date the Privacy Policy was last modified. This Policy was last modified on August 16, 2016.
